Use the ZMachinery onboarding kit I authorize ZMachinery to withhold the first $250 from the pay period following my first eligible payment for onboarding expenses. Of that amount, $125 will be applied to the cost of my background check and drug test, and the remaining $125 will be applied to required PPE and equipment. Any deduction remains subject to the signed agreement and applicable law. Provide my own compliant gear I decline the kit and will obtain, maintain, and bring all PPE and equipment required by my signed agreement before reporting to work. I understand that I must also pay for my own ZMachinery-required background check and drug test in accordance with Field Nation contractual standards. Failure to arrive properly equipped may result in removal from the assignment, contractual penalties, or nonpayment for affected contract work, only as allowed by the signed agreement and applicable law.

06 Screening disclosure & consentZMachinery may require a current background check and drug test before assigning work that carries buyer, end-client, site-access, safety, or Field Nation screening requirements. Screening may be administered through Field Nation, Checkr, a collection site or laboratory, or another authorized screening provider. Screening information will be used only for lawful assignment eligibility, client or site compliance, safety, and recordkeeping, and will be disclosed only as permitted by law. Any separate notices, authorizations, identity verification, or provider forms required by law or the screening provider must also be completed; this acknowledgement does not replace them.
I consent to participate in a ZMachinery-required background screening conducted by an authorized screening provider. I understand that, subject to applicable law and separate required disclosures, the screening may include identity verification, criminal-history records, driving records, or other lawful records relevant to assignment eligibility. I understand that I may request information about the screening provider, dispute inaccurate information, and receive any notices and rights required before or after an adverse decision. * I voluntarily consent to a ZMachinery-required drug test administered by an authorized collection site, laboratory, or screening provider, including collection and analysis of the required specimen and limited disclosure of the result for lawful assignment eligibility, client or site compliance, safety, and recordkeeping. 07 Agreement and SOP acknowledgements
